A. Applications for a permit from the board of health shall be made to the board of health in the form required.

B. Prior to issuance of a business license or permit for any of the above referenced businesses or activities, approval of the board of health is required as well as proof of issuance of a board of health permit. (Ord. 8/27/2002B (part))
